I have downloaded a copy of, read and understood all that is contained in both the ARTICLES OF PIRACY and the BY-LAWS. While a member of Ye Notorious Krewe of the Peg Leg Pirate, I agree to abide by all Articles contained therein. I agree to receive occasional emails from Ye Notorious Krewe of the Peg Leg Pirate. I understand I can unsubscribe at any time.

I submit this voluntary applicaton for Stowaway/Membership into Ye Notorious Krewe of the Peg Leg Pirate, Inc. and agree to abide by all Krewe By-Laws, Articles of Piracy, Parade waiver, and standards of behavior. I understand that this application will be reviewed by the Board within thirty (30) days of receipt to the Membership Chair.

I cerrtify that all informaiton provvided is true tot he besto f my knowledge. I am solely responsible for my actions during Krewe functions and will not hold Ye Notorious Krewe of the Peg Leg Pirate, Inc. responsible for any resons for harm to my person or possesions.

I, the undersign, hereby grant Ye Notorious Krewe of the Peg Leg Pirate, Inc. permission to use, adept, modify, reproduce, distribute, publicly perform, and display in any form now or later developed, my image or visual likeness, my name and/or personal information. I release and hereby agree to indemnify, defend, and hold harmless Ye Notorious Krewe of the Peg Leg Pirate, Inc. and its agents from any and all claims, I or third party, may have now or in the future for invasion of privacy, right of publicly, copyright infringement, defamation or any other cause of action arising out of the use, reproduction, adaptation, distribution, broadcast, performance or display of personal information.

I wavie the right to inspect or to approve any item that may be created using my personal information and waive any claim with respect to the eventula use to with the personal information may be applied during my tenure with Ye Notorious Krewe of the Peg Leg Pirate, Inc., without limitation or expectation.

I understand that membership dues must be paid annually by May 31st to maintain membership in good standing. All payments will be by check or online through the member only section of our website. I agree to pay a penalty of $50.00 if my check should be returned by the bank for any reason whatsoever. The $50.00 will be in addition to the amount of the original check (dues or any purchases).

I am of full legal age and have read this release and are fully familiar with its content. By typing my FULL NAME below, is full and unqualified consent to the terms of this release.
